What is Support Coordination?

Support Coordination is an NDIS funded support, provided in the Capacity Building category of a plan.

Firstly, a Support Coordinator can assist you to understand the funded supports included in your plan. Secondly, find your way through the wide variety of services and supports available in your community. In short, throughout your plan period guidance will be available to achieve your specific goals by:

• Assisting with set up and management of services and documentation.
• Setting out the scheduling of supports in your daily life and engaging with providers.
• Collaborating community, mainstream and funded services to meet all support needs.
• Also provide tips and tricks to maximise funding utilisation and ensure comprehensive report sharing at plan review.

Above all, the main responsibilities of Support Coordination encourages effective, independent engagement of support services over time. As a result, when you begin to realise your goals, you will reduce your need for support coordination.
